RECORDS REQUESTS

Weber Fire District is committed to transparency and public access to information, under the Government Records Access and Management Act. (GRAMA). You or your representative can request access to public records or your own personal care records.

 

NOTE: Utah Code § 63G-2-204 (GRAMA) requires a person or authorized representative making a records request furnish

the government entity with a written request containing the requester's name, mailing address, best phone number, and a description of the record requested that identifies the record with reasonable specificity.

To submit a request, please see the guidelines and fees below.

Requests

PERSONAL CARE - MEDICAL REQUEST

Pursuant to Utah Code 63G-2, a request for a record classified as private, controlled, or protected may only be granted to the person entitled to access the record.

If you are the subject of the record, or the parent or legal guardian requesting records for a minor child, you must submit notarized documentation sufficient to establish identity and legal right of access to the record. Acceptable documentation may include a valid government-issued ID, along with an insurance card listing both the requestor and the minor child, a court order, or a divorce decree.

If you are not the subject, including attorneys, insurance companies, or other third parties, you must provide documentation establishing legal authority to obtain the record. Requests involving personal, health, or medical information must also include a signed HIPPA Authorization from the subject of the record along with the request form listed above.

In order to access your record or the record of a client, you can use your company letterhead or use this Records Request Form.

ALL OTHER RECORD REQUESTS

For records that do not involve personal care or medical information, including fire incident reports, environmental, or hazardous materials records, studies, or investigations, requesters must submit the request form below with sufficient information to identify the record being requested.

If the requested record contains private or protected information, the requester may be required to provide documentation establishing legal authorization to access the record. Records will be reviewed and released in accordance with Utah Code Title 63G-2 (GRAMA) and may be redacted or withheld as required by law.

For environmental or hazardous material requests, please include a clear description of the information being requested and include the address and/or parcel number(s), if know. 

*NOTE: Fire Investigation records related to open or ongoing investigation are classified as protected records under Utah Code Title 63G-2 and are not subject to release

*MUST INCLUDE

  • Type of incident - Accident, EMS Event, Fire; Environmental, Hazmat, etc.

    • Identify what, be specific.​

      • Cardiac

      • Traumatic

      • Traffic accident

      • Structure fire - Commercial/Residential.

      • Vehicle fire

      • Grass/wildland fire

      • Dumpster/refuse fire

  • Date of service. - Be specific

    • If a range is needed, please specify the incident date and add the range requested in the description.​

  • Location of incident. - Address or Intersection or Coordinates. Include City.

  • Complete names.

  • Birthdate.

*Failure to provide documentation or if information is incomplete, it may delay processing or result in a denial.

    • Investigation Records* $75

    • Environmental Records $25

  • Staff Hourly Rate:

    • If research, manipulation, or preparation of information is required (such as redaction, pixilation, scan time), exceeding 15 minutes, an hourly charge is assessed. 

    • The hourly rate will depend upon the lowest hourly rate of the employee with proper security clearance and/or knowledge required to perform the task.

FAQ'S

  • PUBLIC RECORDS

    • Public records are generally available for inspection by any person upon request, unless otherwise restricted by law.​

      • Fire Incident summaries with personal information redacted.​

      • Response statistics and call-volume data.

      • Fire inspection reports for commercial properties (excluding protected details).

      • Public education materials and fire prevention documents.

      • EMS Billing information may be public when it is aggregated or de-indentified.

      • General hazardous materials response summaries (without sensitive details).

      • Environmental inspection reports that do not reveal security-sensitive information.

      • Public notices related to spills, releases, or clean-up activities.

  • PRIVATE RECORDS

    • Private records contain personal information about an individual, the disclosure of which would constitute a clearly unwarranted invasion of personal privacy. Access to private records is limited to the subject of the record or others authorized by law.​

      • EMS Billing records may be private when it includes personal identifying or financial information without medical details.

      • Fire reports containing personal identifying information of occupants or victims.

      • Names, addresses, phone numbers, or dates of birth of involved parties.

      • Witness statements or complainant information.

      • Facility-specific hazardous materials inventories.

      • Sensitive site plans, chemical storage locations, or response strategies.

      • ​Ongoing environmental enforcement or compliance investigations.

  • PROTECTED RECORDS

    • Protected records contain information that, if disclosed, could reasonably be expected to jeopardize public safety, compromise security, interfere with investigations, or reveal confidential operational information. Access is restricted and may be denied as provided by law.

      • Detailed fire response tactics, staffing, or operational plans.​​

      • Fire investigation records related to suspected arson or criminal activity.

      • Building pre-plans and critical infrastructure layouts.

      • Body-worn camera footage or audio recordings, as applicable.

      • Facility-specific hazardous materials inventories.

      • Sensitive site plans, chemical storage locations, or response strategies.

      • ​Ongoing environmental enforcement or compliance investigations.

  • CONTROLLED RECORDS

    • Controlled records primarily consist of medical, mental health, or psychological information. These records are subject to additional access restrictions and may only be released in accordance with GRAMA and applicable federal laws, including Health Insurance Portability and Accountability Act (HIPAA). Access to controlled records is strictly limited requires valid HIPAA authorization and/or other documentation as required by law.

      • EMS patient care reports.

      • Medical assessments, treatment records, and transport documentation.

      • EMS billing records are controlled if they contain medical or health information.

      • Audio recordings or narratives containing protected health information.

      • Medical evaluations or exposure assessments related to hazardous materials incidents

      • Health records associated with contamination or exposure investigations.

  • BILLING RECORDS

Billing records may be classified as public, private, or controlled under GRAMA depending on the content of the record. Billing records containing medical or treatment information are classified as controlled records and are subject to additional access restrictions under state and federal law.

The classification of records and determination of access is made in accordance with Utah Code Title 63G-2 and other applicable state and federal laws.
